Vision and Autism: Understanding Visual, Movement, and Sensory Challenges

Autism Spectrum Disorder (ASD) is a neurodevelopmental condition that can affect how a person processes sensory information, perceives visual input, and communicates and interacts with the world around them.
To understand some of the challenges children with ASD may experience in daily life, it is helpful to look at how the brain processes information from three key systems: the vestibular, sensory, and visual systems. Together, these systems help us stay balanced, move with control, understand where our body is in space, and interact smoothly with the world around us.

What is the vestibular system?
The vestibular system is responsible for informing the brain about the position of the body in relation to gravity, supporting postural control, and maintaining stable vision during movement.
What behaviours can we observe when vestibular processing is affected?
Under-responsivity (hyposensitivity)
The child may seek lots of movement and enjoy activities such as spinning, swinging, or jumping. They may seem to need more movement than other children, appear less aware of their body position, and often seem “fearless” during physical activities.
Over-responsivity (hypersensitivity)
The child may avoid movement activities such as swings or playground equipment, feel anxious when their feet leave the ground, dislike elevators or escalators, and become easily dizzy or uncomfortable with motion.
What is the sensory system?
The sensory system is responsible for receiving and processing information from the environment and from the body. It includes touch, sound, sight, movement, body awareness, and balance. This system helps the brain interpret what we are experiencing at any given moment and allows us to respond appropriately to the world around us.
What behaviours can we observe when sensory processing is affected?
Some children may be over-responsive, meaning everyday sensations such as sounds, lights, textures, or movement feel overwhelming or uncomfortable, leading them to avoid certain environments or situations.
Others may be under-responsive, meaning they may seek more intense sensory input or not notice sensory information as easily as expected.
What is the visual system?
The visual system is responsible for receiving and processing visual information. It also includes visual perceptual skills, which allow the brain to interpret and make sense of what the eyes see. These skills help us recognize objects, understand visual information, and interpret what is happening around us.
What behaviours may indicate a visual perception difficulty?
Children with visual perception difficulties may display a wide range of behaviours at home, in the classroom, and during play. While every child is different, the following signs may indicate that visual processing skills should be evaluated as part of a comprehensive assessment.
Writes off the line when writing
Gets tired very quickly when doing homework
Appears messy or disorganized
Confuses words that look similar
Inconsistent letter size
Skips lines or words when reading
Has difficulty learning to ride a bicycle or scooter
Gets tired easily when copying from the board to paper
Difficulty shifting attention
Does not enjoy playing ball games with others
Letter reversals or mirror writing
Easily becomes disoriented in familiar environments
May struggle to perceive the overall visual context
Avoids eye contact
Focuses on rotating objects, lights, or repetitive patterns
Glances to the sides or uses peripheral vision instead of looking directly
Difficulty maintaining visual attention on specific tasks
These behaviours do not necessarily mean a child has a visual perception disorder, and many can overlap with other developmental or learning differences. However, if several of these signs are present, a comprehensive developmental vision assessment can help determine whether visual processing challenges are contributing to your child's difficulties and identify appropriate treatment options.
Autism and Vision: Why a Routine Eye Exam May Not Be Enough
A standard eye exam is mainly designed to assess visual acuity (how clearly a person sees) and detect eye health issues such as refractive errors or ocular disease. However, in children with Autism Spectrum Disorder (ASD), visual challenges often extend beyond clarity of vision.
As discussed, many of these children may experience difficulties in visual processing, including visual attention, visual perceptual skills, eye movements, and how they interpret and respond to visual information. These aspects are not fully evaluated in a routine eye exam.
A Multidisciplinary Assessment
Dr. Mueller, our neurodevelopmental optometrist, performs a very in-depth evaluation to identify missing or underdeveloped visual and developmental skills in children with ASD. This assessment has three important pillars:
Binocular visual testing
Evaluates visual skills related to eye teaming, such as central-peripheral integration, accommodation, convergence, and overall binocular coordination.
Visual perception testing
Assesses how the child understands and interprets visual information. Areas of focus include visual memory, visual spatial perception, fixation, tracking, reversals, visual-motor integration, and primitive reflexes.
NeuroDevelopmental Movement testing
Identifies potential gaps or delays in early developmental milestones that may affect the development of higher-level visual and motor skills.
After the Assessment: How can we help?
Once a comprehensive assessment is completed and visual challenges are identified, treatment can begin. The main goal of intervention is to help individuals with Autism Spectrum Disorder (ASD) organize and process visual information more efficiently.
Possible Treatment Options
Optical correction and use of filters and/or prisms
This includes the use of glasses to correct refractive errors such as myopia, hyperopia, or astigmatism. In some cases, prisms may be prescribed to support visual alignment, spatial perception, balance, or posture. Tinted lenses or filters may also be used to reduce light sensitivity and improve visual comfort.
Rehabilitative programs
Depending on the results of the assessment, a NeuroDevelopmental Movement program or Vision Therapy program may be recommended.
A NeuroDevelopmental Movement program focuses on early developmental foundations, working on sensory integration, reflexes, and motor patterns that support overall neurological development.
Vision Therapy focuses on higher-level visual skills such as eye coordination, visual processing, tracking, focusing, and visual-motor integration.
Supporting Your Child's Development
Autism Spectrum Disorder affects every child differently, and so do the visual, sensory, and movement challenges they may experience. Understanding how these systems work together is an important step toward helping children feel more comfortable, confident, and successful in their everyday lives.
Many behaviours that are often attributed solely to autism may also have an underlying visual component. Identifying these challenges through a comprehensive developmental vision assessment can provide valuable insights and help guide appropriate interventions that support learning, mobility, communication, and daily activities.
